Behavior Section - Therapist/Clinician initiated the session to provide the individual/client/patient with therapeutic interventions for post-traumatic stress symptoms. Therapist/Clinician met the individual/client/patient at (his or her home) or (the office). The individual/client/patient is experiencing post-traumatic stress symptoms (fear, anxiety, flashbacks, intrusive thoughts, depression, avoidance) (indicate how many times weekly) _-_ times per week. The individual/client/patient was able to respond to the interventions appropriately. The individual/client/patient (denied or confessed) experiencing suicidal and/or homicidal ideations and/or attempts. The individual/client/patient presented (anxious, defiant, calm, agitated, moody, happy, energetic, angry, flat, sad, suicidal, tired, non-congruent or other).
Intervention Section - Therapist/Clinician used the trauma-focus cognitive behavioral therapy modality to explore thoughts and feelings associated with trauma. Therapist/Clinician informed the individual/client/patient that in the session they would use a trauma scale to address traumatic symptoms. Therapist/Clinician assessed the number of episodes the individual/client/patient encounters with re-experiencing traumatic events per week. Therapist/Clinician supported the individual/client/patient in identifying the frequency (1-2x, 2-3x, 4-5x and 6-7x weekly) and the intensity (mild, moderate or severe) of intrusive, upsetting memories of the event. Therapist/Clinician supported the individual/client/patient in identifying the frequency (1-2x, 2-3x, 4-5x and 6-7x weekly) and the intensity (mild, moderate or severe) of flashbacks (acting or feeling like the event is happening again). Therapist/Clinician supported the individual/client/patient in exploring the frequency (1-2x, 2-3x, 4-5x and 6-7x weekly) and the intensity (mild, moderate or severe) of nightmares (either of the event or of other frightening things). Therapist/Clinician supported the individual/client/patient in identifying the frequency (1-2x, 2-3x, 4-5x and 6-7x weekly) and the intensity (mild, moderate or severe) of feelings of intense distress when reminded of the trauma. Therapist/Clinician supported the individual/client/patient in identifying the frequency (1-2x, 2-3x, 4-5x and 6-7x weekly) and the intensity (mild, moderate or severs) of intense physical reactions to reminders of the event (e.g. pounding heart, rapid breathing, nausea, muscle tension, sweating). Therapist/Clinician and the individual/client/patient developed a plan to reduce trauma symptoms.
